I walked into this office to get a medical release form so my Dad could become a patient.  The receptionist would not give me one unless I could tell her who his Doctor was previously.  The next day my sister walked in and a different receptionist gave her a form without question.  Also, there was a disturbing photo of the receptionist hanging from her monitor with a large part of her breasts revealed, which may have made since if I was in a plastic surgeons office, though it would not have been good advertisement.
