Mission Yolo Adult Day Health Center (YADHC), located in Woodland California, was established in 1984 with the mission to assist at risk adults to remain in the community by providing medical, social, rehabilitative and respite services. By accessing the services, participants are able to remain in their homes, retain control of their lives, contribute to the community and avoid premature placement in a skilled nursing facility. Products ADULT DAY HEALTH SERVICES: -------------------------------------- The center assists with personal care, health monitoring, walking, exercising and managing medications while providing needed socialization. Many participants are seniors recovering from illness, such as stroke or heart disease, who may also be facing depression and isolatoin. Younger adults with chronic conditions also benefit from the supervision and therapies offered. The program offers: • A secure and stimulating environment • Small groups with personal attention • Appropriate recreatoinal activities • Family support group CAREGIVER SUPPORT PROGRAM: ----------------------------------------- The Caregiver Support Program assists individual caregivers and families by providing: • Time off • Support groups • Workshops and trainings • Resource information • Counseling and service coordination A resource specialist is available for consultation in person or via telephone. Home consultations are also available.
