This is a FREE program for patients with diabetes, high blood pressure, or high cholesterol to receive monitoring of A1c, blood pressure, and cholesterol.
This program has been in place since 2009, and is sponsored in part by the University Of Colorado School of Pharmacy and a grant from the Colorado Department of Public Health. This clinic is staffed by trained pharmacists and 4th year pharmacy students from the University of Colorado. The objectives of this program are to improve health outcomes for patients with diabetes, high blood pressure, and high cholesterol in the Pueblo community.
We are here to help individuals take control of their health. We are here to educate patients so that they can understand their diabetes, high blood pressure, or high cholesterol, and how to properly manage their medications and lifestyles to optimize their health.
This program is by no means a way to replace physician management of these conditions, but is rather an additional tool to utilize and supplement physician care in between doctor visits. Trained pharmacists and advanced pharmacy students will always contact the primary physician if any concerns in screening results or medication management arise.
Enrollment in the program includes:
- Cholesterol screening
- Foot exam for those with diabetes
- Blood pressure screening
- Blood glucose screening (A1c)
- 6+ Monthly Personal Education Sessions with Advanced Pharmacy Student
- Customize education series based on your needs
- Medication Management tips and recommendations
- Basic Nutrition information
