The South East Grey Community Health Centre is a dynamic health team serving Grey County!
WHO WE ARE:
The community health Centre team has doctors, nurse pracitioners, registered nurses, social workers, a dietitian, physiotherapist, community developer and health promoter.
WHAT WE DO:
We offer many types of health programs and services to members of the community. Our purpose is to build on the existing health care in the South East Grey Community.
WHO CAN BECOME A PATIENT AT THE COMMUNITY HEALTH CENTRE:
People with no family doctor or nurse practitioner.
People who have a family doctor or nurse practitioner that is not close by.
HOW DO I BECOME A PATIENT:
Fill out the "Request for a Family Doctor or Nurse Practitioner" form on the back of this page.
Forms can also be picked up at the Community Health Centre. To have a form mailed to you, call us at 519-986-2222 or 1-855-519-2220.
I ALREADY HAVE A FAMILY DOCTOR. CAN I USE OTHER PROGRAMS AT THE COMMUNITY HEALTH CENTRE?
Yes.
You do not have to be a patient at the Community Health Centre to get help from the dietitian, physiotherapist, social worker, community developer, or health promoter.
A referral from your doctor or nurse practitioner may be needed for some services. Your doctor or nurse practitioner does not have to work at the community health centre to make a referral.
