Went in 5 times to submit the same paper work they said I didn't send the last time I was there. Went in today, just to find out, they yet again didn't "receive" anything. This is the worst office by far!!!!! How did u not receive something when I personally went in 5 times to get it you. BS

I am in need of a surgery because I have a cyst on my ovaries.  Every time I call they always tell me the paper work is on It's way. This has been like a month or two already
