This facility is one of the worst I have ever seen. I filed a written complaint with the Director a year ago, to date no response. My brother's clothing went missing. My sister volunteered to wash his clothing at home but his clothing continued to disappear. We asked the staff but they were of no help. He was left sitting in his wheelchair outside in the sun for several hours not once but twice. He was very sunburned. He fell from his bed and was injured but we were never notified until we went to see him. He was missing from the facility for several hours before they were aware of his disappearance. We received a phone call from Avalon asking if we knew his whereabouts. If possible check other facilities before leaving your loved ones at Avalon. I regret not having moved him sooner. I rated this facility one star because the system with not allow a negative star review.

Most of the nurses are nice with agency's worker! They are very patience and always ready to help us!

So disgusted with staff at Avalon. My mother was a resident at Avalon. I was not happy with the place but it was a place near by and where I could keep close eye or relationship with my mother and be the best advocate for her. I was not able to be her personal care but I was determined to be her advocate. If you ever are in a situation where you have to place a love one in one of this places for whatever reason don't just place them and think they will be take care of them and give them the personal care they need. We must keep and eye at all times and advocate as needed for them. For what ever reason personnel can not notice when a resident is too sick, tired or unable to eat for themselves. (They are too busy on the phones or public media to notice) We have to bring it to their attention. If a nurse is available round the clock why can't they notice when a patient is ill? You have to bring it up to them to please check. This week I had to make them aware of obvious pain my mother was experiencing. Her chart should clearly indicate her diagnosis and make nurses aware . Four months ago, she was diagnosed with cancer and should have known and expected pain . I had to remind each one /shift of her diagnosis to plead for pain medication when it was an obvious constant pain. She was given tylenol which was not going to stop the strong pain. I had to request her food to be pureed, which after talking to a dietitian was done, but no one was able to figure out or report that she was not able to swallow. I requested her to be sent to the hospital for 4 days, by the time they finally did she had a serious pneumonia. That could not be treated. She went through unnecessary pain, because I feel they do not care.

So disgusted with this place. Filthy and dirty. left my husband species in his Porta-Potty all night long. Whatever you do don't leave your loved one there

Short staffed, cnas and aides not careful to sanitize bed pans, don't respond to call lights timely, got dermitis and had a hard time getting my medication, Dr didn't examine me much just checked my heart, some nurses don't even know what is going on and only good staff are occupational and physical therapists. I'd never stay here again. Seems all the good staff left or are leaving.
