SIMPLE PROCESS TO SIGN-UP FOR SERVICE:

FIRST: Call us and give us and give us an idea of your Home Assistance Requirements and Expectations. Give us the Diagnosis of the Patient/Client, Demographics and an idea of the Client’s Personality. Mention to us any Discharge Plans for the Patient. FREE or NO CHARGE, we will Set-Up an appointment for an Intake Assessment Process and for you to interview a possible HCA we will assign to the Case.

SECOND: After achieving a Personality Match between Patient and HCA, last part is Level of Care and other considerations finalized. The Rate of Service and Service Agreement are discuss and explained. Our Service Agreement is an Open-Close Agreement. No Term Commitment. We provide the service only as needed.

THIRD: Service Starts, it is simple. As our commitment to deliver the professional quality service you require, we monitor the HCA. We concern your satisfaction and we guarantee the professional quality of service you deserve.

We are available 24 Hours 7 Days a Week to Provide Professional Quality Care. We listen to your issues and concern regarding the HCA’s and/or the Service of the Agency. They our footsteps to reach service excellence.


LEVELS OF CARE:

Level 1 Care, Normal to Limited Assistance Care. Patient has Normal ADL Conditions. Mobility and Transferring Conditions, or Ambulation of Patient is Normal. Requires Limited Assistance.

Level 2 Care, Extensive Assistance Care. Patient ADL requires Extensive Assistance. Ambulation and Transferring requires Assistance to Extensive Assistance.

Level 3 Care, Total or Dependent Care. Patient ADL is Totally Dependent to HCA. Patient is Bedridden. Uses Hoyer Lift, Fed through a Tube or a Machine. Patient is Critical Assistance Care or Hospice Assistance Care.
