The Community Care Coordination program can
1. provide a knowledgeable healthcare advocate to assist you with navigating through the health care system.
2. help connect you with resources for food, utilities, medical supplies, life skills, counselling, or other needs.
3. help you obtain medical insurance.
4. help you find a doctor if you do not have one.
5. help you obtain prescription medications.
