Story

Clifton Park Family Medicine of Community Care Physicians, P.C. is a full service family medicine practice devoted to providing high quality, compassionate medical care to you and your family. We treat patients of all ages, from newborns to geriatrics. Our practice has served the medical needs of families of Southern Saratoga County since 1992.

Patients have continued to elect our practice for their and their family's health needs. Our patients are confident, and trust our long-standing commitment to their good health and the health of their families. We are proud to make a difference in the health of our community.

Our physicians are all board certified in family medicine and we have two experienced Family Nurse Practitioners who serve as integral parts of our care team. Our caring staff is dedicated to meeting your needs as efficiently as possible and strive to provide the highest customer service and care quality.

To better serve our community we offer early morning Walk-In from 7-9am and have late hours Monday through Wednesday. These extended hours better meet the needs of our patients. Our providers are on call 24/7, and we are affiliated with Community Care Physicians’ Urgent Care Centers across the capital region.

Our physicians’ interests and skills include pediatrics, adolescent care, women’s health, men’s health, holistic medicine, sports medicine, weight management, adult care, geriatric and preventive care. We truly are the specialists you will never outgrow.

At our new facility on route 9 we continue to offer even greater services! For your convenience, phlebotomy (lab blood draw), medical imaging, and physical therapy services are offered under one roof.

Clifton Park Family Medicine welcomes new patients and participates with most major insurance companies.
