Story

Family Medicine Center provides comprehensive healthcare with continuity and compassion for family members. We are committed to enhancing the delivery of the highest quality primary care to residents of Southern Ocean County through care coordination, efficiency, and practice transformation activities. Acting as your medical home, our goal is to provide holistic healthcare and assistance in navigating our complex healthcare system.

We are working hard to make changes in our practice to become a Patient Centered Medical Home (PCMH). This means our medical practice successfully meets new national standards to provide you with responsive and higher quality health care. As part of this commitment, we have a dedicated care coordinator at our practice. She will help make sure you get the care you need, at the right time and place. As part of a PCMH, you will receive more coordinated, comprehensive, personalized health care that supports you and strengthens your relationship with your doctor. Good medical care means more than merely treating episodes of illness. Periodic health survey for the protection and early detection of illness is equally important. This consists of a review of one's routine tests and consultation with the physician. All patients should be seen annually for preventive care.

Patients who have not been seen in the past three years will be removed from our active file and will need to re-establish their patient status by making an appointment for a complete physical examination. Once medical records are current, then inactive patients will be placed in the practice's active file.
