Patient Centered Medical Home

The providers and staff of Mountville Family Practice are pleased to announce that we have received recognition from the National Committee for Quality Assurance and have been certified as a Patient Centered Medical Home.

The Patient Centered Medical Home is a model of care emphasizing care coordination and communication which leads to higher quality care at a lower cost. It centers on a true partnership between the patient and the team of health care givers lead by our providers.

We qualified for this designation by showing that we use evidence-based guidelines for prevention of disease and chronic disease management, use an electronic medical record to enhance your care and follow up, and coordinate your care across the spectrum, helping to navigate the course when other specialists are involved in your care or if you are admitted to a hospital or other health care facility.
