Services Offered:
*Newborn Examinations (2-4 days old)
*Well Check Visits (1-2 weeks, 1, 2, 4, 6, 9, 12, 15, 18, 24 month exams)
*Annual Examinations for ages 3-18 years of age
*Same Day Sick Visits (ages 0 to 20 years and 364 days old)
*Provider interviews (for new patients or parents new to the area)
*Immunizations
*Wart removal
*Asthma & Allergy consultations
*ADHD consultations
*Chronic illness consultations
We offer mental health counseling for AHCCCS patients one day a week (Wednesdays from 9:30am-4:00pm).

What to bring to your appointment(s):
New Patient Appointment:
*Health Insurance Identification Card. (We must have your insurance card at initial visit. If you do not have it, you will have to reschedule)
*Photo Identification for Mom/Dad/Legal Guardian.
*Immunization Records.
*Medical Records/Immunization Records from prior providers (if you have them).
*Medical Records from Urgent Care/Emergency Visits (if you have them).
*Medication List (Including any medications your child is allergic to).
Well Child Check Visits:
*Immunization Record "Blue Book" (We will record shots given for your personal records in the Blue Book. If you do not have a Blue Immunization Book and would like one, they are available at our office for purchase).
*List of medications you child is currently taking.
*Names of any medications your child is allergic to.
Sick Visits:
*Urgent Care/Emergency Room Records (We can request them if you do not have access to these).
*List of medications you child is currently taking.
*Names of any medications your child is allergic to.
ADHD visits please bring:
*ADHD Medication Bottles.
*Medical Records from prior providers that have treated your child for ADHD.
