I want to lay all previous negative reviews to rest. The facility is EXTREMELY clean. RN and Doctors work around the clock and treat they're patients as they're own patients. Cna's, nurses, doctors, physical therapist devote they're life and more to they're patients. In regards to the smell, you will find that in any nursing home because they are there to get care 24/7 and no there is not anything on the wall. We are they're family and family sometimes never come in and it's our job from the time they open they're eyes to the time they close they're eyes for evening. Our facility is more than safe and state laws are met. Dining rooms are cleaned 3 times a day, showers are given daily, laundry is passed out several times a day and any minor repair or major repair is taken care of instantly. Activities director in facility daily. Murals are placed on the wall to provide serenity and safety measures are taken very seriously. Doors are kept open for safety unless resident requests to have door closed. The owner , Hr, administration,  maintence take care of any issue that arises. Again, i please ask that if you are considering a facility, please come see our facility first hand :) we offer much more than a review :)

Adequate. Think Motel 6 of rehab. I'm only judging on the physical building and rooms. Not a place I'd want to go, but, then, I don't need rehab .... Currently. 

The few interactions I had with staff were pleasant and helpful .... Not really enough to say much more than that.

The owner has his father, mother-in-law, and sister-in-law all here as patients and they also get excellent care.  I have worked at Choice Rehab Care for four years and have worked in the health care industry for over 25 years and I find Choice Rehab to be one of the finest facilities I have ever seen or worked with.  They have received 9 consecutive "Excellent" Quality ratings from the Arizona Dept. of Health and with the last three have scored 99 out of 100 on two and 100 out of 100 on the very last state survey.  That is a very difficult distinction to receive.  I find the facility clean and home like with no odors.  I find a very compassionate staff who work very hard in a difficult industry.  I find a very skilled rehab staff who return a very high percentage of their patients back home much stronger and better able to function.  They employ many military and non military staff and I am proud to be associated with this fine facility.  Come in and visit and you will not be disappointed.

My mother's things were stolen from her before her passing.. clothing, her phone, her tablet. Her care there for those few months was awful.

The quality of care at Choices is deplorable  My mother had two infections that required isolation, probably due to the lack of cleanliness in her room.  We had to keep asking for her room to even be swept, let alone deep cleaned.  

The aids were unfriendly and, for the most part, never had a smile or friendly word.  

We've moved my mother and should have done it a lot sooner!
