I called asking when I would receive my brother's death certificate.  He passed away two weeks ago.  I was told they don't automatically send them out that you have to apply for it and the cost is $40.  I'm trying handle his affairs and can't do anything without that death certificate.  I worked for AZDHS for 13 years and I have not problem with saying the state of Arizona sucks when it comes to customer service.

I'm confused because I called in for a client to contact someone for birth records and I could not get a hold of anyone. Is this place a scam?? If this is the department of vital records, i must say that as a citizen and as a lawyer I am very disgusted

18th and Adam's location is by appointment only. It would've been nice if the website said that before I drove 40 miles to get here. Highly unprofessional for a statewide service.
