This is the WORST public office I have ever had to deal with. Poorly ran by Dr. Nate Smith and staff Phyllis Nicholes in the Birth Certificate Dept. I submitt a request for a copy of cert. 3 mo. ago and have NOT recived it. When you call for assistant. NO ONE WILL ANSWER !!!  Even if you get another dept for help, NOTHING.  Cashed my check and no birth Certificate

I know that we as massage therapist are responsible for renewing our license which I had personal family issues that should not matter when it comes to the department doing their job. My career as well as my home is in jeopardy because I haven't worked in almost a month because how long its taken for the department to process my paper work yes it is unacceptable for me to be late but I have done everything I'm suppose to do it also ridiculous that out of the ENTIRE department you guys only have ONE PERSON working g on massage paper work WHO IS NEVER AT WORK WHEN I CALL! UNACCEPTABLE! I HAVE a child and we are about to put out on the streets because I've been waiting to go back to work! Y'all expect so much from us and you guys don't even care!!!

My wife need finally divorce paper and we send mail first may 12 after I did only vital check payed 39$ and we get nothing ! My wife call almost every day no one answer it!  How u can be so slowly this job almost 3 mouths nothing
