As the premiere 5 STAR MEDICARE RATED skilled nursing facility in Mesa, Arizona, Desert Blossom Health and Rehabilitation provides compassionate and personal skilled care 24 hours a day. We specialize in providing rehabilitation services onsite to help our residents regain their independence and return to their homes.

Our guiding principle is to provide outstanding and innovative healthcare in an environment that remembers the importance of the individual and family. We maintain the highest degree of care which allows us to have extremely low re-admission rates to the hospital and very successful outcomes in therapy. Each year every skilled nursing facility is visited by the State Health Department, searching for problems that may affect the health of patients. The Arizona state average is 9 deficiencies per facility, however Desert Blossom received 0 deficiencies in our most recent inspection! Often the transition to a skilled nursing facility is challenging. At Desert Blossom Health and Rehabilitation, we strive to ease the process by providing an environment where residents and guests feel informed and comfortable. We offer newly remodeled private and semi private rooms to give our residence a home-like atmosphere. Our new, state-of-the-art gymnasium and therapy equipment allows our residents to receive the highest therapy care.

We provide the highest degree of care including a team of qualified professionals help residents recover from surgery, injury or serious illness. We have a wound care doctor and two wound care specialists who round weekly. Residents are cared for 24 hours a day with RNs on duty during that time. .
