Just to give you a little background on myself, I’ve worked as a CNA since 1984 and I was trained to report abuse. From my experience in  Florida, if you report abuse to DCF against certain facilities, adult protective services will come after you and place fake charges against your license. I was sent to work at The Forum, at 3001 Deer Creek County Club Blvd in Deerfield FL, by United Nursing Services. I noticed seven months of a nurse abusing a patient on the third floor. I reported it to the nurse from the agency I worked for, the patient reported the abuse to her too. She told him to move to another facility because she doesn’t want to lose the contract. 

      On June 28, 2017, the Patient asked for pain medication from the nurse at the Forum, he had surgery on June 9th, 2017, and was in pain, she came to his apartment with some paperwork and a medicine cup. She shakes the cup in his face and told him he won't get the pain medication until he signed the papers she had in her hand. He cried and and called his grand-nephew who was in New York at the time, and his grand-nephew told him to sign the papers. He told me the nurse wanted him to sign for a psychiatrist that he refused, she took him off his sleeping pills and replaced it with psychiatric drugs and miss diagnosed him with dementia. On June 29th, 2017, the patient told me the nurse pushed pills into his mouth and he woke up in the emergency room at Broward North on Sample Rd, he believed she was trying to kill him. He asked me to call the abuse hotline, which I called at about ten a.m. I spoke to Shayla I-473, she said they can do what they want, there is nothing to report, case closed. About five hours later, Tonya Jackson from DCF came and cursed out the patient and told him I stole $60,000. He started to cry and she asked me to sign a paper and keep my portion. I called all of the phone numbers for DCF, they hung up the phone and laughed at me. Two days later, Cheryl George, a DCF investigator left a card at my door. I called, she asked me to come to 201 W. Broward Blvd, she told me Tonya Jackson used a $60,000 case that she is investigating from a different agency and put the charge on me. She said the Forum reported that I stole their patient out of the hospital, and I have two more charges on me. I called the entire Southeast Region, and reported the abuse to Lara Johnson, Mr. Clark, Maria Shady, and many more, they all laughed at me and told me that I was the only one under investigation, and that I must get legal aid. I then called the attorney general office they told me to call Mr. Fern Melvin. I did and a woman answered the phone, she said she is his personal secretary and  that I cannot talk to him. I'm surprised to see the workers from a government agency behaving like this, being that they are doing the work that taxpayers pay them to do. Please look into this problem, DCF failed the kids and the elderly.

Smart ass Richard something likes to pretend he is the boss. No respect at all. Just went by my GPS to locate this building only to be greeted by chimney sweeper smart ass with a chip on his shoulder wanting to flex his muscles

My issue is with TANF. I'm waiting on a Disability decision and unable to work. There are exemptions where I can receive TANF and not have to look for work but my worker at CareerSource won't work with me on this. Very upset!!
