I went in to have a tooth pulled and he did an acceptable job doing that.

Problem is, when I went back, he told me to fill out a form for him to bill the insurance company prior to work being done.  I looked at the form and it was completely blank.  My response to him was that he needs to fill out the form before I could sign it.

He started getting snippy with me by telling me that it's a matter of trust that I should just sign it and I should trust him to fill out the information correctly.  There was no way that I was doing that, especially not when it comes to my medical insurance coverage.  He wouldn't let it go, so I asked him if he wanted me to just leave because I wouldn't sign the document first.  He said nothing, so I just left.

Also keep in mind that the office is unfinished and that with the ice in the parking lot, no one thought to put any kind of rock salt down so that people wouldn't fall and break a tooth on the way in.  There was water pouring from the roof and leaving icicles and a huge ice puddle right as you walked in.

You can honestly do better.
