Our services include Child Health, Maternal Health, WIC, Family Planning, Breast and Cervical Cancer Program, General Clinic, Nurse Family Partnership, Care Coordination for Children/ Pregnancy Care Management, Health Check/ Health Choice, Community Care Plan, Health Education, Home Delivered Meals, Home Health, Environmental Health, and Animal Control.
