Dear Employee316, or whatever your real name is,
I find the letter you, or someone else in your organization sent to me (and I assume everyone else who is a patient of Dr. Kolia) to be completely akin to a scam telephone call.
For openers, the envelope did not have a return address, the letter did not identify the name of the person preparing this document, their position in this organization, or their signature.
Having patients receiving this letter respond to an unknown person with a “Gmail” email address is so unprofessional.  One would expect at least an email address with the organizations domain name, and directed to an official of this organization.  
But given that there is an organization calling itself the “Heart Center of Southern Maryland, L.L.P., and being a patient of Dr. Kolia, I do have a few questions to ask of you.
Your letter states: “We have learned that your personal information, including name, address, birthdate, and telephone number may have been compromised.”  What was the other personal information?  
From what I have gained from other sources, there was no additional information released other than what I indicated above; which is in the public domain and available easily online using a Google search.  Also, this information was NOT released from Dr. Kolia’s office, but was RECEIVED by them, indicating those responsible for this release of personal information came from another source.  As stated in your letter, this information was done during the transition process of Dr. Kolia’s practice from your organization to MedStar.  One can then assume that this patient information was in your possession prior to Dr. Kolia’s move, and released by YOU to Dr. Kolia’s office or to someone else; hopefully MedStar. 
I am responding to you using my Gmail account.  I do this to remain somewhat anonymous to you as you have done to me.  But please respond to my questions.  Was my Social Security Number released?  Was my medical chart information released?  Was my medical insurance information released?  What else might have been released that I should be aware of that would cause me to inform the three credit bureaus and request a fraud alert.  A simple yes or no answer to the above questions will do, as I’m sure everyone involved is suffering the same loss of personal information.
