Horrible experience with Horizon Subacute Center. The staff missed her scheduled bath several times, even after I talked to the nurse manager. I had to talk to the administrator to get the staff to give her baths for the scheduled twice a week baths. I don't believe they ever cleaned her dentures in the near 30 days she was in Horizon. She developed a UTI while in Horizon and had a fever for several nights and I am unaware of the nurses doing anything but treating the fever until the infection got really bad, ultimately developing a sepsis (according to Clovis Community Hospital). She had to be transported back to Clovis ER and returned to Horizon after a few days of IV antibiotics. Then Horizon business department called me telling me that she owed a copay that she in fact did not owe. Horrible experience with Horizon. I give them a D- in taking care of my mom.

The staff doesn't know what they're doing. The administrator Silas Acosta is the rudest, most unprofessional person I met my entire life, he's a bully. he's new. A couple months. ( staff turnover). He's never there, doesn't know anything, about anything. The nurse director didn't know anything, was smoking cigarettes 20 feet from the door, and was drinking a big starbucks, looking at me all confused when I was asking her questions. The administrator screamed me, and said he was gonna call the police, when i was telling him his staff was abusive to me. So, it's ok for him and his staff to abuse me? Yell at me in front of my mother? I asked for his boss, he said too bad, your not gonna know. This is the worst experience I've had dealing with my elderly mother. One day the staff says something, the next doesn't. No one knows what they are doing around there. It's horrible, a nightmare!! Her medications are all mixed up, wrong meds given, the nurses are not NOT R.Ns. they do the know anything. Wrong foods, that can interact with her meds!!! That can kill her!! It's pretty on the outside/ inside. But the staff turnover is high, according to staff, and even one of the Dr's. That come once a week, told me, no one cares there. As soon as you leave, the patient is left all isolated, and they don't watch if they eat or drink, they just don't care. If you want your family member to eat, check in them,you gotta go yourself and feed them! And they lost her dentures!! Her hair is on knots!! They don't bathe them. The staff is low pay, so they just don't care!! If I wasent there everyday asking to give her water, juice, food, This and that, check her oxygen level, etc..etc..She'd be dead by now!

My grandmother stayed here for the last two years of her life. The staff was always so sweet and helpful.. I have some great memories with my grandmother thanks to Horizon. The food is good too!

Family member was there, didn't get admitted until 2 days before being discharged (after 3 weeks of being there). Medication wasn't given on time or not at all. Nursing staff didn't even know what they were giving. The call button could be on for hours with no one coming in. Hardly ever took to the bathroom. If I wasn't there my family member would never have help out of bed, into bed, showered, taken outside, or to activities. Never had a warm meal served to her room. She ate in her room because no one ever took her to the dinning room. It was the worst experience ever. I wouldn't recommend this place to my enemies!
