Story

Saint Agnes Hospice
Founded in 1978 by Sister Raphael McGrath, the Saint Agnes Hospice program was the first of its kind in the San Joaquin Valley. Since then, terminally ill patients and their families have been able to call upon the resources of Saint Agnes Hospice to offer peace-of-mind care and support, pain management and comfort over curative care at home.

Working under the direction of the patients physician, the Saint Agnes Hospice care team assists with care and provides pain management and symptom control, including emotional, psychological, social and spiritual support for patients and their families.

Our compassionate and professional interdisciplinary team comprises physicians, nurses, social workers, hospice aides, bereavement counselors, spiritual care providers and volunteers. Other Saint Agnes Hospice team members may include pharmacists, dietary counselors and therapy professionals who help enhance the patients quality of life.

Hospice services are reimbursed by Medicare and Medi-Cal, as well as other insurance plans. No patient is denied hospice care based on his or her insurance or financial circumstances. Contributions allow Saint Agnes Hospice to provide services regardless of a patient's ability to pay.

Saint Agnes Home Health
Saint Agnes Home Health was founded in 1975, and specializes in skilled intermittent home health services, with a goal to help patients live safely and independent at home. Medicare-Certified, and accredited by the Community Health Accreditation Program (CHAP), we consistently meet or exceed the national standards for quality care.

Services provided include; skilled nursing, physical, speech and occupational therapy, home health aide, and medical social services.

Saint Agnes Home Health combines clinical experience and excellence, with compassionate and personalized care in the comfort of your own home. In collaboration with your physician, all aspects of your home health care are managed by the primary nurse or therapist assigned to you, achieving life and health balance as you return to your everyday activities safely at home.
